Our Wound Assessment service is provided by a Registered Nurse with over a decade of clinical experience and extensive wound care knowledge, delivering compassionate, evidence-based care right in your home. 🔍 What’s Included: Comprehensive Wound Evaluation We assess the wound’s size, depth, drainage, signs of infection, and surrounding skin integrity. Whether it’s a pressure injury, diabetic foot ulcer, surgical wound, or skin tear—our goal is to support your healing and prevent further breakdown. Circulation & Sensory Check We screen for reduced blood flow or nerve impairment—factors that can affect healing—especially in clients with chronic conditions. Photo Documentation (Optional) With your permission, we can document the wound’s progress and communicate updates with your primary care provider or care team as needed. Care Recommendations & Dressing Guidance We’ll guide you on appropriate dressing types and techniques for your wound care needs. We can also provide wound care and dressing changes. Please note: clients are responsible for providing dressing materials. If needed, we’re happy to discuss cost-effective, locally available options or help you select products that suit your wound and budget. Education & Prevention Strategies You’ll receive practical tips for protecting the wound, preventing infection, and reducing risk of future wounds—tailored to your lifestyle and health condition.
