Whether you're dealing with a new concern, a chronic condition, or just want a routine wellness check, we offer both general and focused nursing assessments tailored to your needs. 🩺 What’s Included: General Health Assessment A full-body review of your current health status, including: Vital signs (blood pressure, heart rate, respiratory rate, oxygen saturation, temperature) Pain assessment Skin integrity and hydration check Functional ability and mobility review Mental status and cognitive screening (if applicable) Focused Assessment For specific concerns such as: New or worsening symptoms (e.g., swelling, pain, dizziness) Post-surgical follow-up Medication side effects or compliance Monitoring of chronic conditions (e.g., hypertension, diabetes, heart failure) Concerns referred by a physician, nurse practitioner, or care coordinator Care Coordination & Recommendations We’ll share our observations and recommendations with your primary care provider upon request and help you plan next steps—whether it’s scheduling further testing, seeing a specialist, or adjusting your routine.
